Incident reports, from a near miss to a reportable event, with the assessment, investigation and actions attached. This is the record a regulator asks for.

Step by step

    Log the incident with New Incident. Record Date, Time, Category, Type, the Client and Staff involved, the Location, and what happened.

    Set the Priority — Critical, High, Medium or Low — and assign it to someone with Assignee. An incident with no assignee is an incident nobody is working on.

    Record what was done immediately. The actions taken at the time are what the Commission asks about first, and they are hardest to reconstruct later.

    Check the NDIS Reportable flag and the reportable category. Rostery assesses reportability and suggests an answer — read the assessment rather than relying on memory of the rules, and override it if you disagree.

    Watch the notification deadline on a reportable incident. The five most serious categories must reach the Commission within 24 hours; an unauthorised restrictive practice has five business days unless it caused harm.

    Record the notification once you have made it, so Reported to NDIS and the notification date are on the record. Pending Report on the dashboard counts the ones that have not been.

    Complete the investigation and the follow-up. Tick Follow-up Required while it is open, and record the outcome and resolution notes as they land.

    Use Private Ticket for an incident that should not be visible to general staff — a matter involving an employee, for instance.

    Close it only when the actions are actually done, not when the form is full.

  10. 10

    Export what you need: Download as PDF or Word for a single incident, or Download Register (.xlsx) for the whole register, which is what an auditor usually wants.
