        What a plan reassessment report is for

        Before a participant’s plan is reassessed, the NDIA asks their support coordinator for a plan reassessment and evaluation report. It tells the NDIA how the plan has gone, which supports worked, where funding went, and what the participant needs next. A clear report with evidence attached gives the delegate what they need to decide on reasonable and necessary supports.

        Make each section count

        • Goals in the participant’s words. Rate progress the way they see it, using the NDIA’s five-point scale.
        • Budget use against time. Spending 80% of a budget at the halfway point, or 20%, both need explaining. Section 7 shows the gap.
        • Justify every recommendation. Say why it is needed, what outcome it should achieve, and any risks or effect on other supports, with reports attached.
        • Be specific about barriers. Name the barrier and what you did about it, not just that there was one.

        The NDIA asks that you complete reports with the participant and make sure they know what you are telling the Agency. For day to day records, use the billable case note template. In Rostery, goals, notes and budget use are already on the participant’s file, so the report starts filled in.

        Questions people ask

        What reports does a support coordinator have to provide?

        The NDIA lists three: an initial plan implementation report, completed 8 weeks after the plan begins or after you accept the request for service; a mid-term implementation report, completed on a regular basis; and a plan reassessment and evaluation report, completed before the participant’s next plan reassessment. The timing for each participant is set out in the request for service.

        Is this the NDIA’s official template?

        No, but it follows it. The sections, rating scales and status options come from the NDIA’s “Support coordination – progress report” template. We added the recommendations section from the NDIA’s list of what progress reports should include, and optional budget and spend fields to show how fast funding is being used.         What should a support coordination report include?

        The NDIA says progress reports should generally include a summary of supports provided, how they helped the participant work towards their goals, links to mainstream and community supports, barriers and the strategies used, any risks to the participant or others, evidence relevant to deciding reasonable and necessary supports, and justification for any recommended additional supports, including the proposed outcomes and any risks or impacts on other supports.

        How do I submit the report?

        Support coordinators submit reports as attachments in the my NDIS provider portal. The NDIA asks you to submit them yourself rather than asking the participant to send them.

        Will reporting slow progress lead to cuts in the participant’s plan?

        The NDIA’s template says how far the participant pursues their goals is up to them, and that it won’t use a report of less progress than hoped to reduce or remove supports when the plan is reviewed, unless the participant’s disability support needs have changed.

        Is the safety section an incident report?

        No. The NDIA template says the safety section is not a replacement for incident reporting. Registered providers must still meet their obligations to raise concerns, and if there is an immediate risk you should contact emergency services.
